A big part of the reason for our high costs is that almost every player in the health care system has staff or whole departments devoted to documenting why SOMEONE ELSE should be responsible for the cost of any given procedure/event/prescription. This is mentioned in the article, but it probably deserves greater emphasis. It’s a great argument for single-payer.
